World class research takes place at Leicester’s Hospitals

Work is still needed to improve services for patients with conditions of the oesophagus, report medics from Leicester’s Hospitals who completed the world’s largest review into treatment of these conditions.

Professor Janusz Jankowski, from Leicester’s Hospitals Gastroenterology Department led a multi-national team of researchers who looked at nearly 12,000 papers on the care and treatment of Barrett’s Dysplasia and early-stage Oesophageal Adenocarcinoma, both conditions of the oesophagus.

The findings, which have been published online in the medical journal Gastroenterology, show that improved provision of endoscopy equipment, more minor endoscopic surgery, and more tissue sampling are needed.

“The key messages to emerge from this process are that the endoscopic equipment needs to be good and that endoscopic surgery can be better than the more risky open surgery,” said Professor Jankowski. “In addition, there needs to be more and larger samples of tissues taken so that the pathologist can make sure that no early cancers are missed.”

Professor Jankowski and his colleagues have drafted recommendations for the prevention of high-grade Dysplasia and early Adenocarcinoma in patients with Barrett’s Oesophagus. “We have come up with simple ways to improve care in one of the commonest pre-cancerous conditions which, if left unmonitored or untreated, can develop into a cancer that is usually fatal.

“Most of our findings are relevant to clinical practice and should be used immediately to guide clinical activity. In addition, areas where we were not able to reach agreement indicate where future clinical research is likely to most productive.”
